Evaluation Copy Request

1. Read this page thoroughly and enter the information accurately.
2. Students can request their evaluation sheets by submitting this form.
3. If this request increases the marks due to any errors earlier, the student will receive payment made towards requesting the evaluation copies, and a revised marks card will be issued.
